Do I need an insulator between the heat sink and the transistor?
Often yes. The metal tab of many SOT-93 power transistors is connected to the collector, so if the heat sink is grounded, use an insulating mica or silicone pad with insulating bushings. Apply thermal paste on both sides of the pad.
