Which filament diameter does this tube suit?
A 2 mm inner diameter tube is designed for 1.75 mm filament. 3 mm filament will not pass through it.
Can the PTFE tube go right up to the hotend heater block?
No. PTFE is not suited to the high temperature of the heater block. It should end in the cooler upper section of the hotend, usually held by a push-fit coupler.
