Why does the output not go high by itself?
The output is open-collector, so it can only pull the line low. Connect a pull-up resistor from the output to the supply rail that should represent a logic high.
Can I use it with 3.3V microcontroller inputs?
Yes. If the pull-up resistor is connected to 3.3V, the output swings between ground and 3.3V and can drive a 3.3V logic input. The same method works for 5V logic.
